Comments: Since the age of 10 this 28-year-old man has had multiple progressive dark red papules on the lower half of the body associated with episodic heat-triggered acral pain, decreased sweating, dry skin, abdominal pain and an impaired quality of life. He had relatives with chronic burning pain in the extremities, bowel disorders, red purple papules, psychiatric and cardiac diseases. The clinical suspicion of Fabry disease was confirmed with low alpha-galactosidase A activity in blood and a mutation in the alpha-galactosidase A gene. Additional findings included microalbuminuria, perypheral neuropathy, vascular tortuosity in conjunctiva and retina as well as cornea verticilata.. Fabry disease is a rare X-linked disorder caused by deficient activity of the lysosomal enzyme alpha-galactosidase A. Progresive accumulation of the substrate globotriaosylceramide in cells throughout the body leads to major organ failure and premature death. Enzyme replacement therapy with agalsidase alpha leads to significant clinical benefits in patients with Fabry disease, and treatment is likely to alter the natural history of this disorder.

Comments: This 12-year-old girl developed an itchy eczematous eruption around her umbilicus while wearing this belt with a metal and probable zinc-containig buckle. The reaction began to settle down 2 days after removing the belt.

Description: 46 years woman complainig of sever pain around the umbilicus +nausia vomiting worsening gragually from 10 houres ago she had this umbilical hernia for at least 15 years after her last labor but never had such pain tha diagnosis was incarcerated umbilical hernia followed by emergency laparatomy and resection of incarcerated and edematous and cyanotic omentum
Comments: This 46-year-old woman complained of severe pain around the umbilicus associated with nausea and vomiting worsening gradually over the preceding 10 hours. She had an umbilical hernia for at least 15 years following her last pregnancy and delivery but never experienced swelling or pain of the hernia. She underwent emergency laparatomy and resection of an incarcerated umbilical hernia and edematous and cyanotic omentum.

Description: symmetric thickening and hyperpigmentation of creases and bony prominences
Comments: This 3-year-old boy had a history of thickened hyperpigmented plaques with scale on his elbows, knees, and the top of his feet since shortly after birth. He also had intermittent blistering of his feet. His 5-month-old brother and father had a similar eruption. His father reported flexural involvement and blisters at birth. A skin biopsy showed epidermolytic hyperkeratosis.

Comments: This healthy vigorous newborn developed several grouped yellow vesicles and bullae which ruptured with minimal trauma. A skin culture grew Staphylococcus aureus, and the child improved quickly with oral antibiotics and local wound care. No one in the famiy had impetigo, boils or other cutaneous infections, nor was there a history of similar infections from the nursery.
